- Purpose of the Assignment
- The purpose of this assignment is to involve students in the comprehensive community needs assessment. Students will develop/utilize tools to identify perceived factors (both negative and positive) that impinge on the population’s health. This information will then be used to develop and implement strategies for health promotion.

- Student Approach to Assignment
- For this assignment, my clinical group and I reviewed the nursing process and identified the process we took to provide care to our community group. We assessed our population, found a deficit, planned, attempted to implement, and then evaluated how our implementation would have worked and why it did not.

- Applies appropriate knowledge of major health problems to guide nursing practice
- Example: During the assessment and planning of our community project, we assessed their knowledge of physical activity and how it ties into the overall health of an individual. When we asked the residents of the nursing home how often they believe they should be exercising, and how often they do exercise, both answers were significantly lower than the recommendation. Physical inactivity can have lasting effects, and may lead to obesity and other complications.

The regular exercise can also help older adults maintain their independence. As a community of independent living for older adults, this was especially important for our community.

- Example: We established parameters and methods for evaluating the outcomes of regular physical activity in our community. We would have given them a baseline test, and a questionnaire at the end of the course to test if their physical activity levels had changed during the process. We wish we could have identified this, but we were no longer granted access to the site after our first meeting of the fall semester. We also would have tracked their blood pressures and see if there had been a decrease.

- Example: Research in regards to the social sciences helped to provide information about the mental state of older adults based on services provided by their housing. The studies found the sense of community created by a housing unit that engages residents create people to be more resilient mentally. These individuals have a higher perceived purpose in life and lower rates mental health issues.

They provide services to older adults, including meals on wheels and physical exercise classes. We also acknowledged the use of expanding our aggregate to include community centers in the area. This way there are more staff members and we can reach older adults that live at home as well, not just the retirement community.

We organized physical activity meetings with our group once a week, due to time constraints. This would allow the residents to become comfortable with the recommended amount of exercise, so they could continue to implement once we were gone.
